I created this to track my own health record. I figured a template would help others with health issues. It can be used for anyone including spoonies, and people who are well. I hope that this helps people keep track of their health and wellness.
Also, some of the inspiration is from notion ambassador Luisa. She inspired me on a few charts that were modified in a way that I was able to make use of her formula. So thank you Luisa!
How can I duplicate the template?
